Healthy teeth and gums are important to your overall wellness. East West Bank offers coverage through Cigna to help you maintain your smile through regular preventive care and treatment for any dental problems that may arise.

Overview

You can enroll in dental coverage as a new hire, during Open Enrollment, or if you have a qualifying life event. To see your premiums and enroll, log in to the UKG website.

Medical-Dental-Vision Opt Out

You can waive East West Bank health care coverage if you can provide proof of other coverage (such as your Medical ID card). You cannot opt out of medical coverage, and then choose dental and vision coverage only. If you enroll, you must select coverage for medical, dental, and vision.

Cigna Dental Care DHMO

You must receive care from your designated Primary Care Dentist (PCD) or have your PCD coordinate your care with another in-network provider.

Cigna Dental PPO

You have the flexibility to choose any dentist and receive benefits. However, you will pay less if you visit a provider in the Cigna network because in-network providers have agreed to charge lower, negotiated rates.

Cigna Dental PPO Buy-Up

Similar to the Dental PPO, but pays a slightly higher level of benefits and includes orthodontia coverage for children up to 19 years old, as well as implant coverage.

If you enroll in the Cigna Dental Care DHMO, you will be issued an insurance card. If you enroll in the Cigna Dental PPO or Cigna Dental PPO Buy-Up, visit mycigna.com to quickly view ID cards (front and back) for the entire family.

Find a network dentist

If you enroll in the Cigna Dental Care DHMO, You must choose a Primary Care Dentist, who will coordinate all your care. If you enroll in the Cigna Dental PPO or Cigna Dental PPO Buy-Up, you may choose to see any in- or out-of-network dentist you’d like, but you’ll generally pay less when you stay in network.

Use your dental benefits wisely

Here’s how to make the most of your dental benefits:

Choose a network provider.

Each time you need dental care, you have a choice of providers. Selecting a Cigna participating dentist will ensure you receive the highest benefits from your plan. To find a provider, go to the Cigna website.

Present your plan identification.

Give your provider's office the information they need to bill your insurance. The group number is #3333253.

Submit for a pretreatment estimate.

You should submit a request for a pre-treatment estimate for procedures and services your dentist believes will exceed $300 (procedures such as crowns, inlays, bridges, and periodontics). For more information, call Cigna at 800.244.6224 (available 24/7) or visit the Cigna website.

Check your claim status.
